Service Description

Important: This fee covers only the blood draw procedure itself – the professional collection of your blood sample. It does not include the cost of laboratory testing. Lab testing fees are billed separately by our partner laboratory (Quest or LabCorp) or are paid directly by you at the time of service. How Payment Works: 1. Insured Patients using Medicare, Medicaid, Private Insurance - Quest/ LabCorp bills your insurance directly. You are responsible for any deductible, copay, or coinsurance per your plan. Your $25 Blood Draw Fee Due at time of service. 2. Uninsured / Cash-Pay Patients - You pay Lonestar Verify Diagnostic Lab's discounted cash rate for each test ordered. We provide a price list upon request. Your $25 Blood Draw Fee is separate for your lab test and is due at time of service. 3. Patients with a Doctor's Order - has the option for Quest or LabCorp to bill your insurance for your lab tests or you can pay our discounted cash rate based on your coverage. Your $25 Blood Draw Fee is separate for your lab test and is due at time of service. 4. Direct-Access Patients with No Doctor's Order - You pay Lonestar Verify Diagnostic Lab's discounted cash rate for each test ordered. We provide a price list upon request. We Accepted the following Payment Methods: Credit Card (Visa, Mastercard, American Express, Discover) Debit Card FSA (Flexible Spending Account) HSA (Health Savings Account) Money Order Important Notes for Patients ✅ Your $25 blood draw fee is due when we draw your blood. We cannot bill insurance for this fee. ✅ We do not accept cash or personal checks. Please bring a credit/debit card, FSA/HSA card, or money order. ✅ If you have insurance, Quest/LabCorp will bill your insurance for the actual lab testing. You are responsible for any out-of-pocket costs per your plan. ✅ If you do not have insurance, we will provide you with our discounted cash price for each test before we draw your blood. No surprise bills. ✅ We accept most major insurance plans including Medicare, Medicaid, Blue Cross Blue Shield, Aetna, Cigna, United Healthcare, and others. Call us to verify your specific plan.
