Patient Lab Assistance Program
(PAP)
We offer over 400 commonly ordered blood tests, such as A1C, CBCs, Fasting Lipid, Liver Function Panels, Triglycerides, Glucose and more through our Patient Lab Assistance Program (PAP). Some of the tests cost as little as $5, which may be less than the cost of your insurance co-pay.

A patient medical health treatments may be hampered due out-of-pocket lab fees and cost, especially if they do not have health insurance or their insurance does not provide adequate coverage for lab tests. If you are uninsured or underinsured, out-of-pocket lab fees and costs can be a barrier and delay to your medical treatment. It is your responsibility to pay for lab tests before your doctor can prescribe you any medication, even if they wanted to. If you're paying for these tests out of pocket, they can be very costly, which has the worst possible outcome: Patient's don't get the medical treatment they need because of the high cost of lab testing. Patient's might not discover what is wrong with them until their medical issues are much, much worse, which simply causes more health complication and issues.

In addition to receiving the medical services you require, ordering lab tests online through our Patient Assistance Program (PAP) will also provide you with the lab services you require at a cost that is affordable. We believe that purchasing lab work shouldn't require you to make other financial compromises or that money shouldn't be a hindrance to better healthcare. You should feel secure knowing that you're getting the medical services you require fast, effectively, and without the unnecessary cost of purchasing lab tests.

To get started, you would need your physician to complete and sign the attached order form below and have their office fax your lab order form to us. (The order must be written and signed by a your physician.)

Someone from our office will contact you to schedule your lab draw or specimen collection appointment within our clinic. Patients can pay for their lab tests online or in person at the clinic. The patients lab test results will be faxed to the patient's provider. (We do not provide medical advice or analyze your results. Please contact your physician for any follow-up information about your lab test results.)

Our clinic is partnered with Quest & LabCorp, which both offer leading healthcare diagnostics and accurate test results with a quick turnaround time. Once the specimens have been received, your physician will receive results in about 2-5 days. Not only is this an affordable way to prioritize your health, but it is also a straightforward process. Our clinic charges a lab draw and processing fee of $15 plus the cost of the discounted labs collected, with tests costing as little as $5.

Print the lab order form and list of available tests list or call our office for a emailed copy. You may also order online and have your Physician fax your order to us

Have your provider complete the Physician information section of the order form and have their office fax or you may simply fax a copy of the order form to our office. Our Fax number is (903) 765-7618

Lab Order Confirmation
Once we have received and process your lab order form request, a copy of your lab requisition will be emailed to you. We have provided the option for patients schedule to their lab appointments online, our office will also call you to confirm your appointment time and date. Patient's will need to print out with their lab requisition and bring the form with them to the clinic. Your blood work will be drawn at our clinic located at 415 W. Kilpatrick St, Mineola TX 75773 and sent to the testing lab.

Patient Information
Complete the Patient Information section of the order form and be sure to provide your email address.

Make Payment
Order payments are now accepted through our invoicing system, Square. We will then email you an itemized invoice of your requested lab order. Upon receipt of the payment invoice, simply click the link to review your lab order and click the “make payment” button, to complete your lab order payment. Once your payment has been paid, your lab order will be submitted. We will then email you a copy of your lab requisition. If you would like to pay by cash, check or money order, you may do so at our clinic, please bring the completed lab order form , signed by your physician with you to our clinic. ** Please note there is a $15 lab draw and processing fee for each order. This fee is the same not matter how many test are ordered.

Lab Results
​Once your lab specimens are sent to the testing lab and completed, your Physician will receive a faxed copy of your lab results within 2-5 days, upon receiving the completed results back from the testing lab.
